Key Responsibilities • Provide person-centred support and care to children and young people in line with their individual support plans. • Assist with daily routines, personal care, and medication administration where required. • Engage young people in educational, recreational, and social activities both within the home and in the wider community. • Safely transport young people to appointments, activities, and school. • Maintain accurate records and contribute to the review and development of care plans. • Work collaboratively with colleagues, families, and external professionals to ensure holistic support. • Promote independence, choice, and inclusion for all young people.
