Key Responsibilities • Provide direct care and support to children and young people, assisting with daily routines and personal care needs where required. • Engage young people in a variety of activities, both within the service and in the community, promoting their social, emotional, and educational development. • Contribute to the creation of individualised care plans, ensuring they are person-centred and reflective of each young person's needs and aspirations. • Maintain accurate records and documentation, contributing to the overall effective management of care. • Work collaboratively with colleagues, families, and external professionals to ensure a holistic approach to care. • Act as a positive role model, fostering independence, self-esteem, and decision-making skills among the young people you support.
